Blueprint Arranges Sale of Skilled Nursing Facility in Illinois

CRESTWOOD, Ill. — Blueprint Healthcare Real Estate Advisors has brokered the sale of Symphony of Crestwood, a skilled nursing facility in the Chicago suburb of Crestwood.

A joint venture between a public REIT and a private equity investor sold the property to an undisclosed seller.

Symphony of Crestwood is a multistory facility built in 1974 and is the largest long-term care facility by licensed capacity in the area, though the number of beds was not disclosed. The property underwent recent renovations and upgrades and features specialty programs such as cardiac care, tracheostomy care, secure dementia care and on-site dialysis.

Ben Firestone and Michael Segal of Blueprint brokered the transaction.
