NORTH OLMSTEAD, Ohio — Monticello Asset Management, through one of its investment vehicles, has originated $37 million in first lien debt financing for Clermont Nursing Realty LLC, a skilled nursing owner based in the Central Ohio city of North Olmstead.

LOS ANGELES — Housing & Healthcare Finance has arranged a $6 million HUD refinancing for a 99-bed skilled nursing facility in the greater Los Angeles area. The name of the facility was not disclosed.

IRVINE, Calif. — Lancaster Pollard has closed 13 separate refinancing transactions totaling $99.1 million for Granite Investment Group, an Irvine-based investor.

WESTCHESTER, N.Y. — Harborview Capital Partners, a commercial real estate finance, equity and advisory firm, has arranged a $19.2 million bridge-to-HUD acquisition loan and  $1.5 million line of credit for a skilled nursing facility in the New York City suburb of Westchester.

NEW YORK CITY — New York-based Monticello Asset Management, through one of its investment vehicles, has provided $69.3 million in acquisition financing for a four-property skilled nursing portfolio in Massachusetts.

NEW YORK CITY — Greystone has provided a $59.8 million HUD-insured permanent loan for The Pavilion at Queens for Rehabilitation & Nursing, a skilled nursing facility in the Queens borough of New York City.
