PAONIA, GLENWOOD SPRINGS and ROCKY FORD, Colo. — Seniors housing owner/operator Grace Healthcare has officially exited all Western U.S. states with the $15.3 million sale of its final three skilled nursing communities in Colorado.

COLUMBUS, Ohio — Lancaster Pollard has arranged both the sale of and acquisition financing for Columbus West Park Nursing and Rehabilitation Center, a 100-bed skilled nursing facility in Columbus’ Westgate neighborhood.

AUGUSTA, Ga. — Senior Living Investment Brokerage (SLIB) has arranged the $3.7 million sale of Amara Healthcare & Rehab, a 213-bed skilled nursing facility in Augusta, along the border of Georgia and South Carolina.

MORAGA, Calif., PLEASANT HILL, Calif., and PHOENIX — Grace Healthcare has sold Grace Healthcare of Moraga, Grace Healthcare of Pleasant Hill and Grace Healthcare of Phoenix for a total of $14.7 million. The dispositions mark the owner-operator’s exit from the states of California and Arizona.

LYNWOOD, Calif. — JCH Senior Housing Group has brokered the sale of Bloomfield East, a 130-bed skilled nursing facility in the Los Angeles submarket of Lynwood, for an undisclosed price.
