TAMPA, Fla. — Atlanta-based seniors housing investment company AdCare Health Systems Inc. (NYSE MKT: ADK) has agreed to purchase a 55-bed skilled nursing facility in Tampa for approximately $4.8 million. The name of the facility and seller were not disclosed.

SAN RAFAEL, Calif. — Reliant Management Group LLC has purchased Northgate Care Center, a 52-bed skilled nursing facility in San Rafael near San Francisco, from Meridian Foresight Management for $4.5 million.

LEONARD, Tenn. — Senior Living Investment Brokerage (SLIB) has arranged the sale of Leonard Manor, a 79-bed skilled nursing facility in Leonard approximately 60 miles northeast of Dallas, for $1.2 million.

SHREVEPORT, La. — Care Capital Properties Inc. (NYSE: CCP), the skilled nursing REIT that spun off from healthcare giant Ventas in August, has completed its first major transaction.

BIRMINGHAM, Ala. — HealthSouth Corp. (NYSE: HLS), a Birmingham-based provider of post-acute healthcare services, has agreed to acquire CareSouth Health System Inc., an August, Ga.-based home health provider, for $170 million.
