MILLSBORO and SMYRNA, Del. — Blueprint Healthcare Real Estate Advisors has brokered the sale of two skilled nursing facilities in Delaware for $54.1 million.

METHUEN, Mass. — Tryko Partners has acquired Methuen Health & Rehabilitation Center for $7.5 million. Tryko immediately rebranded the facility as Cedar View Rehabilitation and Healthcare Center and launched a $2.5 million renovation plan.

NEW YORK CITY — Meridian Capital Group, a debt broker based in New York, has arranged $135.5 million in loans for Continuum Healthcare to refinance a six-property portfolio of skilled nursing homes in New Jersey and Pennsylvania.

FORT WORTH, Texas — Blueprint Healthcare Real Estate Advisors has brokered the sale of Southwest Nursing & Rehabilitation Center and West Side Campus of Care, both skilled nursing facilities in Fort Worth, for $27.5 million.

LIBERTY, Mo. — Senior Living Investment Brokerage has arranged the $2 million sale of an undisclosed skilled nursing facility in the Kansas City suburb of Liberty.
